- Promote optimal healing, prevent complications, and improve patient’s quality of life by providing comprehensive and effective care for acute and chronic wounds. Our advanced therapies (e.g. negative pressure wound therapy) promote timely and efficient healing by implementing evidence-based wound care strategies. Reduce risk of infection through proper wound cleaning, debridement, and antimicrobial treatments (which also aids in minimizing the risk of chronic wound progression). Address underlying causes of disease that would contribute to delayed healing (e.g. diabetes, venous insufficiency, pressure-related injuries, nutrition, circulation). Allevaiate wound-related pain and discomfort using appropriate analgesics and dressing techniques. Promote patient education and empowerment including techniques for home management. Restore mobility and function by promoting wound closure and minimizing scarring.
- Ensures holistic, patient-centered care that improves health outcomes and reduces the burden of wounds on individuals and the healthcare system.

Transitional Care Management (TCM) is a CMS Program designed to decrease risk and increase continuity of care for patients after discharge.

Transitional Care Management (TCM) is a CMS Program designed to decrease risk and increase continuity of care for patients after discharge. TCM program coordination are potentially billable encounters. The general workflow includes a care manager contacting the patient to discuss the program and setting up an appointment with their PCP to be completed soon after discharge. According to CMS, requirements for TCM Services include:
- The services are required during the beneficiary’s transition to the community setting following particular kinds of discharges
- The health care professional accepts care of the beneficiary post-discharge from the facility setting without a gap
- The health care professional takes responsibility for the beneficiary’s care
- The beneficiary has medical and/or psychological problems that require moderate or high complexity medical decision making
